Personal Information Collection Statement

We endeavour to ensure that all our collection, transmission, storage and usage of personal data shall be carried out in compliance with the Personal Data (Privacy) Ordinance, Cap 486 of the Laws of the Hong Kong Special Administrative Region as amended from time to time.

Statement of Purpose:

Users will have to provide personal information for record and the purpose of research and provision of online mental health services(“Services”) upon registration.

Personal data collected from you may be used by the Living with Heart platform for:
i.) conducting evaluation and research on the Living with Heart platform;
ii.) enabling the provision of the Services;
iii.) following up and handling complaints and enquiries;
iv.) making such disclosures as required by applicable laws, rules and regulations
v.) any other purposes directly related to the purpose for which the personal data were originally collected

Voluntary Provision

If you are a user of the Living with Heart platform, personal data is collected from you to enable the Living with Heart platform to provide Services to you and conduct research. Provision of your personal data is entirely voluntary, but we shall indicate to you the personal data or information without which we shall not be able to provide you the Services.

All personal data collected and held by the Living with Heart platform will be kept confidential, but where disclosure is necessary for the Living with Heart platform to (a) comply with any statutory or legal obligations or requirements, or (b) provide the Services to you, or (c) carry out the original purpose, or a directly related purpose specified above, for which the personal data were collected, those data may be provided to the following parties
i.) Law enforcement agencies, or other governmental, statutory or regulatory authorities, institutions or organizations
ii.) Data hosting service providers, the Living with Heart platform backend service providers, cloud service providers and other service providers involved in research, promotion or provision of the online mental health service provided by the Living with Heart platform
iii.) Any person who is acting for or on behalf of or jointly with the Living with Heart platform in research, promotion or provision of the Services;
iv.) Co-investigator of effectiveness of the Living with Heart platform, AIA Group Limited.

Classes of transferees

All the personal information collected in this study will be strictly protected that only researcher(s) involved in this study and the Joint Chinese University of Hong Kong – New Territories East Cluster Clinical Research Ethics Committee will be able to get access to the data for research analysis and ethics review purpose. Your data may also be transferred to AIA International Limited if you are their members or corporates’ clients. The data will be transferred to AIA International Limited only for a health economic research under the governance of Joint CUHK-NTEC Clinical Research Ethics Committee.

Rights of access and correction and contact detail

You have the right to verify whether the Living with Heart platform, the Chinese University of Hong Kong hold any personal data about you. You also have the right to access any such data; require the above-mentioned entities to correct any personal data relating to you which is inaccurate; and ascertain the above-mentioned entities’ policies and practices in relation to personal data.
